Shopping and Appointments

Accompanying Service Users to Medical, Social, or Personal Appointments as well as assisting with Grocery Shopping.

Activities and Social Integration

Encouraging participation in community events and social activities to promote Mental Well-being and Social Inclusion.

Emotional and Physical Support

Offering Companionship, Emotional Support and Crisis Intervention as needed.

  1. Comprehensive Information Gathering:
    • Psychiatric History
    • Forensic History (if applicable)
    • Risk History
    • Personal Background
    • Health History, Educational Workshops
  2. Initial Assessment by Senior Management: Our experienced management team conducts a preliminary review to understand the individual’s needs.
  3. Face-to-Face Assessment: We visit the individual in their current environment to better understand their routines, preferences, and support needs.
  4. Stakeholder Discussions: Engaging with Ward Managers, Social Workers and Family Members to gather a holistic understanding of the individual’s needs
  5. On-Site Visit and Secondary Assessment: Arranging for the individual to visit our Supported Living Facilities to experience the environment and undergo further assessments.
  6. Detailed Report and Decision: Preparing a comprehensive assessment report outlining the proposed care plan, ensuring the best possible support.
Our Supported Living Accomodations are situated in safe & accessible areas of Birmingham and the surrounding regions.
 
Each property is equipped with modern amenities and security features to ensure a comfortable and secure living environment. Our housing provider works with Registered Social Landlords to ensure our accommodation is fully compliant with HMO regulations.

The Key Worker also assists with registering the Service User with Healthcare Professionals, such as GPs, Opticians, and Dentists, ensuring that all health-related needs are promptly addressed.
